Notice of Availability – Form 1095 – C

In compliance with the Affordable Care Act (ACA), Akytech Consulting, LLC is required to furnish Form 1095-C which outlines the health insurance coverage we offered to you for the previous year.

Under the Paperwork Burden Reduction Act, the 2025 Form 1095-C is now available upon request. The Forms 1095 provide information about the offers of coverage made to full-time employees.

If you would like to receive a copy of your Form 1095-C, you can request the form through one of the following methods:

Requests can be made at any time, and the form will be provided to you by March 2nd , or within 30 days of your request, whichever is later.
